Your online shop has lack of buyer security. Obviously there is a big chance of having an issue with the online purchased item in terms of sizes and quality (reality vs expectation).
You make your customer shoulder the item for return shipping fee and expect to provide tracking number for only 7days, why can't the customer just drop it off to your shop instead or how bout YOU SHOULDER THE RETURN SHIPPING FEE.
Customer's refund is in Old Navy points? Why wont you just send the money back thru how the item was purchased? You are forcing people to buy items from you again with that POOR CUSTOMER SERVICE.
With your expensive items, you have poor customer service, poor buyer protection and your website has poor user interface.

Here are some tips to help you order the correct size. If you don't have a measuring tape, use a piece of string and hold it alongside a ruler.
Measure from the top of the baby's head to the floor.
Note baby's weight to the nearest pound/kilogram.
Measure the fullest part under the armpit.
Measure the natural waist over the belly button.
With no shoes on, measure from the top of the inner thigh down to the floor.
